Summary

One day after the CMS memo, HRSA and SAMHSA sent parallel letters to hospital administrators and grant recipients stating each agency would review its policies, grants, and programs "in light of" the QSSAM and might "re-scope, delay, or potentially cancel" grants depending on grantees' provision of gender-affirming care to minors. HRSA's review explicitly reached the Children's Hospitals Graduate Medical Education program ($367.2 million to 59 children's hospitals in FY2024), tying core pediatric training funds to abandonment of gender-affirming care.

Status

Letters remain operative policy signals; EO-based funding-condition enforcement is constrained by the PFLAG and Washington preliminary injunctions (see section 10), but grant-level implementation continued through FY2026 terms and conditions (see 2026-05 HRSA item; section 9 for terminations).
