Summary
Reporting on August 21, 2025 revealed OMB was withholding $3.1 billion of the $6 billion Congress appropriated for PEPFAR in FY2025, allocating only $2.9 billion, while the administration terminated HIV-prevention programs serving "key populations" — sex workers, people who inject drugs, gay men, and transgender people — restricting U.S.-funded prevention (PrEP, condoms) to pregnant and breastfeeding women. Partner organizations in Kenya and Uganda closed drop-in centers serving transgender clients and laid off providers. (See also section 8.)
Status
Key-population prevention exclusion in effect through FY2026; congressional Democrats' oversight demands and activist protests (arrests at Capitol Hill hearings, March and June 2026) did not restore the funding.
