Summary

Declares U.S. policy not to "fund, sponsor, promote, assist, or support" gender-affirming medical care ("chemical and surgical mutilation") for people under 19. Directs agencies to rescind reliance on WPATH standards; HHS to review Medicare/Medicaid coverage, publish a review of the evidence, and use every available mechanism (including conditions on research and education grants to medical institutions) against providers; OPM to exclude pediatric gender-affirming care from FEHB/PSHB plans; DOJ to prioritize enforcement (including FDA misbranding and consumer-protection theories) and to convene states on private rights of action. Spawned extensive HHS, CMS, DOJ, and FTC implementation through 2025-2026 (see sections 2, 3, 9). EO number 14187; 90 FR 8771 (Feb. 3, 2025).

Status

Core funding-withholding provisions preliminarily enjoined — nationwide since March 4, 2025 (PFLAG v. Trump, D. Md.) and in Washington, Minnesota, Oregon, and Colorado since February 28/March 1, 2025 (Washington v. Trump, W.D. Wash.); appeals pending. Despite the injunctions, the administration pursued the order's objectives through HHS conditions, CMS rulemaking (final Medicaid rule Aug. 13, 2026, 91 FR, doc 2026-16508), DOJ subpoenas to children's hospitals (contested into May 2026), and hospital-level cessations of care (see sections 2, 3, 10, 13).
