Summary
Pursuant to OPM's 2025 carrier guidance (technical guidance and an August 15, 2025 carrier letter), all Federal Employees Health Benefits and Postal Service Health Benefits plans exclude coverage of hormonal and surgical gender-affirming care for Plan Year 2026, for enrollees of any age, across programs covering roughly 11 million federal employees, postal workers, retirees, and family members. An exceptions process applies to enrollees already in treatment. (see also sections 6, 2)
