Summary

CDC allowed its flagship community-based-organization HIV prevention cooperative agreement (PS21-2102, successor to a funding line dating to the late 1980s; 96 CBOs in 31 states, D.C., Puerto Rico, and USVI; $239.3 million distributed 2021-2026) to lapse, and announced it would not renew the "Ending the HIV Epidemic in the U.S. through Community-Based Partnerships" cooperative agreement, cutting off roughly 100 CBOs (three-month extension through September 2026) and rerouting HIV-prevention money through state and local health departments. KFF analysis projected replacement pass-through funding 27-71% below prior direct CBO funding. Many affected CBOs specifically serve Black, Latino, and transgender communities; advocates warned some organizations "won't survive the cuts." (See also sections 2, 11.)

Status

Non-renewal announced July 21, 2026; extensions run through September 2026; transition to health-department pass-through funding in progress.
